A. Description of school and teaching, administrative, or pupil services situation B. Description of the goal(s) to be addressed C. Rationale for your goal(s) and link to reflection, educational situation, and Wisconsin Educator Standards D. Plan for assessing and documenting achievement of your goal(s) E. Plan to meet your goal(s): objectives, activities, timeline, and collaboration *Iniatial Educators submit PDP to Team for goal approval: Goals must be reviewed by PDP Team of 3 members, and approved by at least a majority (2 out of 3) of the PDP team. Note: If the goal approval is done on paper form, make sure to keep the completed goal approval form and submit it along with the PDP to team members for PDP verification. Professional educators do not have to have their PDP goal approved by a PDP team. Please refer to PDP Team section for the team composition or make-up. The PDP team composition (i.e. the team members’ roles) will be the same as the PDP team that verifies the plan upon completion. |
